Privacy Notice
This notice explains what happens to the information you give us through this website. It covers the enquiry forms only. If you go on to become a client, you will be given a fuller notice covering clinical records.
Who is responsible for your information
Solene Marie Daniele Mazu, trading as Soleen Care, is the data controller for information submitted through this website.
Registered with the Information Commissioner's Office under registration C2046927.
You can contact us about anything in this notice at soleencare@gmail.com.
What we collect
When you send an enquiry, we collect only what the form asks for:
- Your name
- An email address or telephone number
- Your preferred method of reply
- Your child's age, if you give it
- Your organisation and role, if you are enquiring professionally
- The service you are asking about, if you select one
- What you have written in your own words about your concern
The last of these may include information about your child's health or development. Under UK GDPR that is special category data, and it is treated with the additional care described below.
We do not use cookies, analytics or tracking of any kind on this website, and we do not collect your IP address for any purpose other than short-term protection against automated abuse of the enquiry form.
The guided questions
The Help Me Find the Right Support pathway works entirely within your own browser. Your answers are used to display a suggestion on your screen and are not transmitted to us, stored, or logged.
Nothing about your child leaves your device unless you choose to send an enquiry, which is a separate and deliberate step. If you close the page without sending an enquiry, we never see anything you entered.
Why we use it, and our lawful basis
We use the information solely to read your enquiry, reply to it, and discuss whether and how we may be able to help.
For ordinary personal data, our lawful basis is Article 6(1)(b) UK GDPR — taking steps at your request before entering into a contract.
Where your enquiry contains health information, our additional condition is Article 9(2)(h) UK GDPR — the provision of health or social care — read with Schedule 1, Part 1, paragraph 2 of the Data Protection Act 2018. Our processing is subject to a duty of confidentiality.
We do not use your information for marketing, and we do not sell or share it with anyone for their own purposes.
Information about children
Most enquiries concern a child who cannot consent on their own behalf. We rely on you, as a parent or person with parental responsibility, submitting the enquiry.
Please send only what is needed for us to understand your concern and reply. There is no need to include detailed medical history, reports or diagnoses at this stage. If we need more, we will arrange a secure way to receive it.
This website is not intended for use by children.
Who can see it
Enquiries are read by the practitioner. They are not shared with anyone else without your consent, except where we are required by law to do so, or where there is a serious safeguarding concern about a child's welfare — in which case we may have to share information with the appropriate authorities without your agreement. We would tell you we were doing so unless telling you would increase the risk to the child.
How long we keep it
Enquiries that do not lead to any service are deleted after 12 months.
If you go on to become a client, your enquiry becomes part of your clinical record and is retained under the separate retention period set out in the client privacy notice.
Where it is stored
The online enquiry form is not yet active. If you contact us by email, your message is received in the practice's email account, provided by Google, which may process this data outside the United Kingdom.
Your rights
You have the right to:
- Ask what information we hold about you and receive a copy
- Ask us to correct anything inaccurate
- Ask us to delete it, subject to any legal or clinical obligation to retain it
- Ask us to restrict how we use it
- Object to our using it
- Ask us to transfer it to another provider in a portable format
To exercise any of these, contact us at soleencare@gmail.com. We will respond within one month.
If you are unhappy with how we have handled your information, you can complain to the Information Commissioner's Office at ico.org.uk or on 0303 123 1113. We would prefer you raised it with us first so we have the chance to put it right.
